Documentation

CRSS will need to collect documentation for new referrals from the support coordinator/case manager assigned to the individual.

Documentation needed may include: 

📄 Psychological Evaluation

📄 Current Physical & List of Medications

📄 Supports Intensity Scale (SIS)

📄 Virginia Individual Developmental Disability Eligibility Survey (VIDES)

📄 Current Person Centered Plan (ISP)

📄 Behavioral Support Plan
